Heart to Home

Program Goals

 

The overall goal of Heart to Home is to provide safe and quality personal care, supervision, and protection while maintaining dignity and respect of each and every resident in our care.        


Specific program goals include:

  • Assisting residents in personal care activities such as dressing, grooming, and bathing.   
  • Assisting residents in appropriate socialization, including productive and independent leisure time, one-on-one interaction, and group activities. 
  • Assisting residents in understanding and expressing their emotions and feelings, focusing on building good self-esteem and a sense of self worth.     
  • Encouraging and helping residents in maintaining involvement with family members and other significant people in their lives.     
  • Accurately assessing and responding to the unique and changing needs of each resident.                                                                           



staffing Levels and Training

Heart to Home has at least one staff on duty at all times. The ratio of staff to residents shall be adequate to meet the needs of the residents as defined by the State of Michigan, Department of Human Services. All Heart to Home staff members are experienced and undergo background checks and fingerprint screening.  All staff at Heart to Home are trained through inservices as well as on the job training.      


Training includes:    

  • Reporting requirements    
  • Cardiopulmonary resuscitation     
  • First Aid       
  • Personal care, protection, and supervision     
  • Resident rights    
  • Fire Safety and prevention    
  • Prevention and containment of communicable diseases  
  • Medication administration   
  • Standard precautions     
  • Nutrition and proper handling/storing of food and drink   
  • Alzheimer’s, dementia, memory care           


All training is completed prior to employment. Refresher courses are required annually and extra training is offered throughout the year.
