Heart To Home - Residential Assisted Living & Memory Care
Room and Board: Heart to Home has two private rooms and two semi private rooms. Each room is furnished with a bed(s) with a 10 inch memory foam mattress, dresser, closet/armoire, chair, grooming mirror and is equipped for cable. The walls are not decorated to allow for each resident to make their space special to them. Residents are encouraged to bring wall hangings, pictures, etc. to make them feel at home. Residents may bring their own hospital bed, TV, and/or chair, if desired.
Wellness Checks: Each resident will receive a complete assessment by a registered nurse at admission to establish a baseline. Residents will be reassessed at regular intervals and as needed to identify if any changes in status are evident. Collaboration with family/representative and physicians will be initiated. Our goal is to intervene early so that injury, illness, and/or hospitalizations may be prevented.
Safety: Heart to Home will train staff/residents to develop a working knowledge and understanding of areas where precautions may be necessary, such as fall prevention, diabetic precautions, pressure ulcer prevention/skin care, emergency plans in case of fire or severe weather, and community safety.
Medication Management: Medications will be administered and monitored by trained staff. Medication administration by authorized staff will occur only when ordered by a physician. The owners of Heart to Home are master prepared registered nurses, licensed in the State of Michigan, and will oversee medication management including collaboration with resident physicians, medication reconciliation, and monitoring the residents response to prescribed medications. All medications are secured in a locked cabinet. Narcotics are double locked and all pills are counted and logged after each shift by both outgoing and incoming staff members. Logs are monitored by Heart to Home management.
Meals/Snacks: Heart to Home provides three meals and two-three snacks per day. Heart to Home strives to serve heart healthy, well balanced, and nutritious meals. We get to know each residents likes and dislikes and incorporate that into our menu planning. If a resident is able and enjoys the process of cooking/meal prep, they are encouraged to do so under the supervision of staff.
Personal Care: Personal Care is provided by trained Heart to Home staff who assist and support the residents in activities of daily living including bathing, dressing, grooming, and maintaining a medication schedule.
Protection: Heart to Home ensures the health, safety, and well-being of each resident, including protection from physical harm, humiliation, intimidation, and social, moral, financial exploitation while in our care.
Supervision: Heart to Home provides guidance in the activities of daily living, maintains a medication schedule, encourages and reminds the importance of activities to be carried out, assisting in making and keeping appointments, and being aware of a resident's general whereabouts.
Activities/Leisure Time: Leisure time is designed to maximize meaningful and productive activities in the home. Family-style home-centered recreational activities include playing cards and board games, listening to music, various arts and craft projects, viewing television and videos, movie nights, and reading books, newspapers, magazines are a few examples. Participation in exercise programs and walking are also offered and encouraged. During the warmer months, the large beautiful back yard allows for gardening, outdoor gatherings, BBQ’s, and other outdoor activities.
Heart to Home strongly encourages community involvement through participation of activities and utilization of various resources in Taylor, MI and surrounding areas via family and friends. Outings with friends and family to local libraries, local shopping malls, restaurants, picnics in a variety of area parks, sporting events, and special community events can be socially beneficial to the resident.
Laundry: All laundry will be maintained by Heart to Home staff on a scheduled and as needed basis.
Outside Services: Heart to Home recommends the utilization of pharmaceutical deliver and the use of visiting physician to service residents in the home. Supportive home care service referrals will be made to outside providers when such services are ordered by a physician including nursing services, physical therapy, occupational therapy, medical social worker, and speech therapy.
